Morphine Versus Hydromorphone. Which Is Better?

In terminally ill patients, subcutaneous hydromorphone and subcutaneous morphine are similarly effective.

Morphine and hydromorphone, a semi-synthetic derivative of morphine, have similar chemical structures, and pharmacodynamic and pharmacokinetic qualities. Mary G. Miller, MD, MRCP, of Churchill Hospital, Oxford, England, and associates compared these opioids in 74 terminally ill patients ("Continuous Subcutaneous Infusion of Morphine Versus Hydromorphone: A Controlled Trial," Journal of Pain and Symptom Management, July 1999;18(1):9-16).
