Home monitoring program offers new support for heart failure patients

Tufts Health Plan and Alere Medical Incorporated announced that Alere will provide new technology and support for in-home disease management for heart failure patients.

Alere, a leader in care management for patients with chronic conditions, has developed an innovative in-home monitoring technology designed to improve care for patients with chronic heart failure. The initiative is expected to enroll several hundred members of Secure Horizons, Tufts Health Plan for Seniors, by the end of the year.
