Insurance carriers routinely cite evidence-based medicine in claim denials to provide an objective basis for the decision. However, appeals can be successful against internally developed treatment criteria.

Insurance executives acknowledge that guidelines are not "binding" on medical reviews. Many applicable state and federal medical appeal review laws require consideration of clinical information submitted by the treating provider.
