Patients rarely tell a healthcare provider that they did not receive an influenza or pneumococcal polysaccharide vaccination when they actually did receive it.

"National surveys of adult vaccination indicate moderate self-reported immunization rates in the United States, with limited validity data. We compared self-report with medical record abstraction for 820 persons aged at least 66 years from inner-city health centers, Veterans Affairs (VA) outpatient clinics, rural and suburban practices," scientists in the United States report.
