SHELTON, Conn.-- HC Innovations, Inc. (OTCBB: HCNV, http://www.hcinnovationsinc.com), a specialty care management company providing high-touch services to patients with complex medical needs, while producing cost savings for HMOs, nursing homes, and insurance companies, has signed a contract with Senior Whole Health to provide medical management services to its members residing in certain long-term care facilities in Connecticut. Senior Whole Health is a leading health care plan providing intensive, integrated case management services to seniors 65 and older receiving Medicare and Medicaid benefits.
As part of the contract, HC Innovations will administer services through its NP Care program, a nurse practitioner medical service that works with physicians to bring the best medical care available to residents in long-term care facilities. Senior Whole Health members residing in certain long-term care facilities in Connecticut will receive around-the-clock hands-on care by an on-site HC nurse practitioner, supported by HC’s proprietary electronic medical records (EMR) system that allows for rapid documentation and capture of accurate diagnosis. Other terms of the agreement were not disclosed.
The NP Care program, driven by state-of-the-art electronic medical record technology, allows nurses and nurse practitioners to evaluate, monitor, and improve a patient’s health. The program provides additional hands-on support and creates a facility-based safety net, thereby improving the quality and consistency of medical care in nursing homes, and preventing unnecessary ER and hospital transfers.
